New Horizons and the COVID-19 Vaccine
SECOND DOSE VACCINE CLINIC NOTICE FEBRUARY 19, 2021
GREAT NEWS! A huge thanks again to Dr. Kwan because he is providing the SECOND DOSE of the COVID Vaccine on February 19, 2021. The opportunity to receive the second dose is open to all New Horizons Staff and Members who have already received the first dose of the Moderna vaccine no later than January 22nd (28 days from February 19th). Even if you received the first dose of the Moderna vaccine somewhere else, you may register to receive the second Moderna dose on February 19, 2021 here at New Horizons.
How to Register:
You must register by providing the following by February 8, 2021, end of business day:

- A copy of your vaccine card with proof of the first dose to your department supervisor.
- Your insurance information ONLY IF:
- This is your first time receiving any vaccine at New Horizons.
- Your information has changed since the first Moderna dose was administered January 15, 2021.
- A photocopy or scan of your insurance card and government issued ID if this is your first vaccine (but second Moderna vaccine) being administered at New Horizons or if your information has changed.
On February 19, 2020 you must:

- Consent Form
- Pre-Vaccination Checklist
- Original copy of your vaccination card
Show up on at your designated time, which will be provided once you register.
Please note: If you fail to show up during this time, you are responsible for securing your own SECOND DOSE through the county or your own healthcare provider.
***Wear a short sleeve shirt on the day of to receive the vaccine. We need to make this process as efficient as possible for Dr. Kwan as he has provided us with this amazing opportunity.
