SUMMARY
Provides on site, in-home/residence training, assistance, supports and education regarding service options, rights and responsibilities. The services are based on the Individual Program Plan and formatted to increase the abilities of the individual to move to a less restrictive program and/or environment. (Maybe a Certified Nurse’s Aide.)

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  1. Develops an activity program based on the needs of the client with the goal of more independence and movement to a less restrictive environment. May also work toward assisting the client to maintain skills the skills that keep them as independent as possible.
  2. Develops a task analysis where necessary that identifies all of the goals/objectives as stipulated in the IPP and ID Team.
  3. Provides activities and supports, encouragement and advocacy to assist the client toward more independence.
  4. Teaches the client appropriate behaviors and attitudes.
  5. Assists the consumer in developing appropriate relationship skills.
  6. Provides whatever moral support and encouragement, instruction, training, counseling, mentoring, and advocacy the person may need to succeed toward more independence.

OTHER DUTIES AND RESPONSIBILITIES include the following:

  1. Completes all paperwork, documentation, or progress notes including assessments in a timely and correct manner.
  2. Attends all scheduled meetings and trainings, produces assessments.
  3. Tracks service hours and completes billing, keeps administration regularly updated.
  4. Conducts self professionally at all times.
  5. Believes in and supports the agency’s mission. Works effectively with coworkers, consumers and others by sharing ideas in a constructive and positive manner; listens to and objectively considers ideas and suggestions from others; keeps commitments; keeps others informed of work progress, timetables and issues; addresses problems. constructively tries to find mutually acceptable and practical solutions; addresses others by name, title or other respectful identifiers; and respects the diversity of the New Horizons workforce in actions, words and deeds.
  6. Performs additional duties, as assigned.

SUPERVISORY RESPONSIBILITIES

This job does not include management and supervisor responsibilities.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The individual must have a reliable vehicle, proof of vehicle insurance, a valid California Driver License, and a driving record acceptable to the agency’s insurance carrier.

EDUCATION and/or EXPERIENCE

A high school diploma or its’ equivalent; minimum of one year of experience working with adults with developmental disabilities is preferred.

LANGUAGE SKILLS

Ability to read, write and interpret documents in English.

The ability to effectively present information and respond to questions in English.  Spanish a plus.
